Huron community Family Health Team

Do you want to see your doctor less?  Do you want to feel better or live longer?  Do you want to have more energy at work, chasing the grandkids or playing on the golf course?  If you answered “Yes” to any of these questions, you need to watch this video.  Even if you answered “No”, watch it any way.  Doctor’s orders!

The Huron community Family Health Team (HcFHT) is a regional, patient-centred, primary health care team set up to provide collaborative health care to improve overall health.

The HcFHT is made up of doctors, nurse practitioners, registered nurses, nursing team, a dietitian, a social worker, psychologist mental health worker and administrative staff whose most important goal is to provide patient-centred care.

The patient is number one - care will be patient centered and the team will collaborate to meet all the healthcare needs by working together.

PROGRAMMING:

Nursing Clinic provides clinical services for Huron community Family Health Team patients: Blood pressure checks; weight checks; dressing changes; ear syringing; suture or staple removal; allergy/other injections; or preventative health immunizations. Appointments are recommended.

Health Care Connect helps Ontarians who are without a family health care provider (family doctor) to find one. People without a family health care provider are referred to a family doctor who is accepting new patients in their community.  To register please call 1-800-445-1822
