Notice to Patients of Dr. Carmen Schmitz
HcFHT is pleased to welcome Dr. Daniel Dalcin who has taken over all patients of Dr. Schmitz.
If you were a patient of Dr. Schmitz and you would like to remain a patient at HcFHT under the care of Dr. Dalcin you must come into the Seaforth office to sign paperwork.
Please note that Dr. Schmitz's office in Bayfield is now closed. Dr. Dalcin is seeing patients at the Seaforth location.
